The CF009 is our application form for certification examinations that are held after public training courses, private training courses, or just a private examination for an individual. Our CF009 form is one of our many document-controlled forms, which means we receive feedback from students, our instructors, and our staff and implement updates quarterly for the form to make more sense.

Candidates fill out their anticipated date of examination, location of examination, their personal information, and provide their preferred spelling of their name for their certificate (should they pass the certification examination). This is how we attempt to keep candidate’s records and information consistent across our membership database and our examination tracking spreadsheets and software. Candidates also provide their school and work experience, typically verified by their employer, to prove that they meet the prerequisites for the respective category they’re applying for.

Staff then decide approval or denial of the CF009 application form based on the information provided. Typically, the most frequent reason for denial is the absence of signatures from the individual. A signature from the employer, supervisor, or international representative is required on page 4 of the application, and a signature from the individual himself/herself are required on page 5 of the application.
